07Where does it hurt?
Lower Back
Step 1
Common symptoms
- Aching or stiffness across the lower back
- Pain that worsens with standing, bending or sitting
- Pain radiating into the buttock or leg (sciatica)
- Numbness, tingling or weakness in a leg
- Pain persisting after back surgery
Step 2
Conditions we treat
The condition pages that cover pain in this area.
Step 3
How we evaluate
Your visit starts with a comprehensive evaluation: your history, a focused musculoskeletal and neurologic examination, and a review of any imaging or diagnostic studies you already have. When it helps to pinpoint the source of pain, we use diagnostic nerve blocks or ultrasound-guided assessment before recommending treatment.
We identify the pain generator — disc, facet joint, sacroiliac joint, nerve root or vertebral endplate — often with a diagnostic block before any longer-lasting procedure.
Step 4
Treatment options
Treatments linked from those condition pages, from conservative care to advanced procedures.
Physical Therapy, Rehabilitation & Functional RestorationPhysical therapy and rehabilitation help patients improve movement, strength, flexibility, endurance, balance, and confidence with activity.
Epidural Steroid InjectionsEpidural steroid injections are image-guided procedures used to place anti-inflammatory medication near irritated spinal nerves.
Selective Nerve Root BlocksA selective nerve root block targets a specific spinal nerve as it exits the spine.
Facet Joint & Medial Branch BlocksFacet joints are small joints at the back of the spine that help guide movement.
Radiofrequency Ablation of the SpineRadiofrequency ablation (RFA) uses controlled thermal energy to interrupt pain signals carried by selected sensory nerves.
Sacroiliac Joint Injections & Diagnostic ProceduresThe sacroiliac (SI) joints connect the pelvis to the lower spine.
Sacroiliac Joint AblationAt APICO Pain and Wellness, Dr. Rany Abdallah offers Sacroiliac (SI) Joint Ablation for patients with chronic lower back or pelvic pain caused by inflammation or…
Basivertebral Nerve Ablation (BVNA)Basivertebral nerve ablation is a minimally invasive treatment for a specific form of chronic low back pain called vertebrogenic pain.
Minimally Invasive Lumbar Decompression (MILD)Minimally invasive lumbar decompression is designed for selected patients with lumbar spinal stenosis and neurogenic claudication, particularly when thickened ligament contributes to narrowing of the spinal canal.
Restorative Neurostimulation (ReActiv8)ReActiv8 is a restorative neurostimulation therapy for carefully selected patients with chronic mechanical low back pain associated with impaired control or function of the multifidus muscles.
Spinal Cord Stimulation (SCS)Spinal cord stimulation is an implantable neuromodulation therapy used for selected chronic pain conditions, particularly persistent neuropathic pain that has not responded adequately to more conservative treatment.
Dorsal Root Ganglion (DRG) StimulationDRG stimulation is a targeted form of neuromodulation that delivers electrical stimulation near the dorsal root ganglion, an important structure involved in sensory signaling.
Peripheral Nerve Stimulation (PNS)Peripheral nerve stimulation delivers electrical stimulation directly to a selected peripheral nerve or nerve region.
Minimally Invasive SI Joint FusionMinimally invasive sacroiliac joint fusion is an option for selected patients with persistent SI joint pain that has been carefully confirmed and has not responded adequately to conservative and interventional care.
Weight & Metabolic Health OptimizationBody weight and metabolic health can influence joint loading, inflammation, energy, sleep, cardiovascular health, and the ability to exercise comfortably.
Bone Health, Balance & Fall PreventionBone strength, muscle strength, and balance become increasingly important with age.
Regenerative Medicine & Orthobiologics (PRP, BMAC & Fat Aspirate)Orthobiologic treatments use autologous materials obtained from the patient with the goal of supporting the body’s biologic healing environment. BMAC & fat aspirate: Coming Soon
Work Injury Evaluation, Rehabilitation & Return-to-Work PlanningWork-related injuries often involve several overlapping issues: identifying the injured structure, controlling pain, restoring function, documenting progress, and determining safe work capacity.
Chiropractic CareAt APICO Pain and Wellness, Dr. Rany Abdallah offers comprehensive Chiropractic Care as part of an integrated approach to managing musculoskeletal pain.
Kyphoplasty/VertebroplastyMinimally invasive procedures that relieve pain from spinal compression fractures caused by osteoporosis or trauma.
Interspinous StabilizationAt APICO Pain and Wellness, Dr. Rany Abdallah offers Interspinous Stabilization as a minimally invasive solution for patients suffering from lumbar spinal stenosis and…Request an Evaluation
